NOTE: Please print this form and
complete to register by phone or mail. Payment in
full is required for all programs. Registrations by mail are
held until the beginning registration date.
PAYMENT: is required in advance by check or credit card
. Reminders will be mailed for balance due. Payments may also
be made at the Member's Entrance by due date on reminder. CONFIRMATION will be mailed upon receipt of program
fee. CANCELLATIONS: Unless noted differently in program
descriptions, cancellations must be received no less than
two weeks prior to date of the program for a refund. We reserve
the right to cancel a registration which is not paid before
the due date on any payment reminder in order to fill that
